Physical Therapist

Job Summary

Provide restorative and rehabilitative Physical Therapy services to participants both at the center and in home settings. Collaborate closely with physicians, rehabilitation staff, and interdisciplinary team members to maximize participant independence and safety. Participate in care planning, deliver and document therapy interventions, supervise therapy assistants, and support care transitions. Engage in training, mentoring, and leadership activities within the therapy department.

Key Responsibilities

• Conduct Physical Therapy assessments and participate in interdisciplinary team meetings to develop care plans.
• Deliver and document therapy interventions including active/passive exercise, muscle re-education, gait and functional training, and various treatment modalities.
• Supervise and train Therapy Assistants in implementing Functional Maintenance Programs.
• Provide training to staff, participants, and caregivers on therapeutic equipment use and care.
• Manage therapy equipment maintenance.
• Communicate participant condition changes promptly to the care team.
• Support smooth care transitions between settings (hospital, skilled nursing, home).
• Participate in end-of-life care coordination.
• Manage direct reports including training, supervision, and mentoring.
• Complete all required documentation and mandated trainings timely and accurately.
• Maintain confidentiality and adhere to policies and quality improvement activities.

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 5 years of progressively responsible Physical Therapy experience
• At least 2 years of Physical Therapy experience in a clinical setting with a frail or elderly population
• Physical Therapy experience within the last year
• Bachelor’s Degree in Physical Therapy, or its US-equivalent evaluation for degrees earned outside the United States
• Experience working with geriatric patients
• At least 2 years of supervisory experience with demonstrated ability to mentor and develop team members
• Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects
• Basic Life Support (BLS) certification

Preferred Qualifications

• Master’s Degree or Doctorate in Physical Therapy (foreign degrees must be evaluated to US equivalent).
