Job Description

Required Certifications & Licenses

• BLS (Basic Life Support): Must be American Heart Association (AHA) certified
• Nursys Verification: Within 30 days prior to start; disciplinary actions require FACIS Questionnaire
• Current Nursing License: Primary source verification within 30 days prior to start; disciplinary actions require FACIS Questionnaire
• Ascension Color Vision Test: Completed prior to start and updated every 3 years
• Ascension Core Competencies: HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly (minimum 80% score, max 2 attempts)
• NIH Certification: Group A updated yearly; Groups B-F updated every 2 years
• Ascension EKG Exam: New edition as of Dec 1, 2023; passing score of 80% required with max 2 attempts
• Ascension Onboarding Requirements Education Attestation: Must be completed pre-hire
• Ascension Nursing Contract Worker Onboarding: Completed onsite prior to start
• Ascension Nova StatStrip: Pre-hire modules for non-allied positions
• MOAB Moderate Risk De-Escalation Training: Required
• Ascension Cerner Test-Out: Cerner system training and test results required
• Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations
• Driver's License/State ID: Must be current, match address on file, and comply with compact license rules if applicable

Health & Immunization Requirements

• MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Fit Test: Required for all travelers; preferred prior to start, can be completed onsite; updated annually
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Influenza Vaccine: Optional but recommended; flu season Oct 1 - Mar 31; exemptions require mask wearing
• Positive PPD/Negative Chest X-Ray & Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest x-ray within one year, and completed TB questionnaire; updated annually
• Health Statement: Statement of good health or physical exam prior to first assignment, updated annually
• 2 Step PPD or IGRA: Required within 30 days prior to start; TB questionnaire updated annually
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Ascension 12 Panel Drug Screen: Completed within 30 days prior to start; includes testing for 12 specified drugs; updated annually

Professional Requirements

• Ascension Profile: Resume or professional profile required
• Professional References: At least two qualifying references prior to first assignment
• Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; completed within 30 days of first assignment and updated annually or after breaks in service >90 days
• EPLS (Excluded Parties List System) Check: Within 90 days of start; rerun if break in service >90 days
• Skills Checklist: Clinical skills checklist completed within one year of start and updated annually
• Education Verification: Verification of relevant education within 90 days of start; rerun if break in service >90 days
• Employment Verification: Third-party verification for past 7 years; updated annually or after breaks in service >90 days
• Confirmation of Assignment: Provided upon initial assignment and updated with each extension

Optional / Additional Information

• Ascension Unit Specific Checklist: Optional for onboarding, must be uploaded within a week by unit leader
• Hiring Manager Evaluation: Post-assignment evaluation (optional)
• Name Change Documentation: Legal documentation if applicable (optional)
