Location: Columbus, Ohio
Employment Type: Long-Term Assignment (LTA)
Shift: Nights, Monday through Thursday, 9:00 PM – 7:30 AM
Contract Length: 13 weeks

Job Description

• Certified Pharmacy Technician position working night shifts.

Required Qualifications

• Valid Pharmacy Technician license in Ohio.
• Minimum of 1 year of Pharmacy Technician experience, including experience within the last year.
• BLS certification issued by the American Heart Association.

Onboarding and Compliance Requirements

Candidates must provide documentation for the following prior to or at onboarding:

Health and Immunizations

• Fit Test
• 2 Step PPD OR IGRA (Quantiferon/Tspot) for TB screening (with chest x-ray if positive, per regional requirements)
• COVID-19 vaccine proof including booster or approved declination/exemption (declination not accepted in CA or IL)
• MMR vaccination proof (2 doses or IgG titer lab report)
• Hepatitis B vaccination series initiated prior to start (titers not accepted)
• Varicella vaccination proof (1 dose or IgG titer lab report)
• Physical/Health Statement confirming fitness to work (exam within last year)
• 10-panel drug screen completed within 30 days of start (specific drugs required)
• Influenza vaccine proof or declination/exemption (optional)

Professional and Background Checks

• Criminal background check (federal, nationwide, county) covering last 7 years, including SSN verification and all known aliases, dated within 30 days of start
• Education verification of highest academic degree or diploma/GED
• EPLS (Excluded Parties List System) report dated within 30 days
• OIG (Office of Inspector General) exclusion check dated within 30 days
• NSO (National Sex Offender) check dated within 30 days, including statewide check for Michigan
• Signed Confidentiality, Conflict of Interest, Code of Conduct, Integrity and Audit Services forms
• Trinity Profile submission including resume, skills checklist (within 1 year), and 2 professional references
• System Access Form completed and uploaded one week before start
• Joint Commission CORE exam within the past year
• Standard I9 form completed by the agency

Additional Notes

• The agency will provide the Trinity Skills Checklist for clinical contractors.
• All required documents must be uploaded into the vendor management system prior to or at onboarding.
