Job Opportunity: Case Manager - Registered Nurse
Location: San Leandro, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Provide case management services including admission criteria, care coordination, discharge planning, disease management, and plan of care.
- Ensure compliance with CMS, HIPAA, OSHA, and Joint Commission standards.
- Schedule includes every other weekend; no float or call required.
- Accepts first-time travelers.
- One return-to-office request allowed.
Required Qualifications
- Valid Registered Nurse (RN) license in California, verified through NURSYS (RN, LPN/LVN, AP).
- Bachelor of Science in Nursing (BSN).
- Minimum 1 year of case management experience within the last year.
- Minimum 1 year of outpatient nursing experience.
- Experience in case management including care coordination, disease management, plan of care, and discharge planning.
- Knowledge and experience with CMS (Centers for Medicare & Medicaid Services), HIPAA, OSHA, and Joint Commission standards.
- BLS (Basic Life Support) certification issued by the American Heart Association.
- PALS (Pediatric Advanced Life Support) certification issued by the American Heart Association.
- Orientation Safety Manual Acknowledgement completed.
Compliance and Documentation Requirements
- Valid photo ID (Driver’s License or Passport).
- Background checks including criminal, sanction, and exclusion screenings within 90 days prior to start.
- Education verification with official documentation.
- Current, unencumbered CA RN license verification (NURSYS).
- Physical exam within 90 days of start, signed by MD, DO, PA, NP, or APRN.
- Fit mask test within 90 days of start (specific mask types required).
- Immunizations and health screenings including:
- Tetanus/Diphtheria/Tdap (within 10 years or signed declination).
- Negative TB test or chest X-ray updated annually.
- Proof of MMR (Measles, Mumps, Rubella) vaccinations or positive titers.
- Hepatitis B positive titer or declination.
- Varicella vaccinations or positive titer.
- Seasonal flu vaccination or declination documentation.
- Drug testing: Oral saliva drug screen within 30 days of start (specific analytes required).
- Competency exams and medication/pharmacology competency as applicable.
- Photo for badge (passport style, no filters).
- Completion of facility-specific orientation and competency modules on first day.
Additional Information
- References and employment verification for the past 3 years required.
- Resume including last 7 years of employment required.
- No float or call responsibilities.
- Schedule includes every other weekend.
