Portland, Oregon (includes Clark County, Washington and surrounding areas)

Contract Length

13 weeks

Schedule

Day shift, 8 hours per day (8:00 AM – 4:30 PM)
5 days per week (40 hours/week)
No weekend requirement
No on-call
No floating between units
No block scheduling or alternative shift patterns (no 4x10s or 3x12s)

Job Description

Home Health Physical Therapist providing skilled care in East Portland, OR and Clark County, WA
Patient ratio 1:1, may visit up to 6 patients per day
Productivity standard: 30 points per day
First-time travelers accepted
Must have own transportation; daily travel radius up to approximately 60 miles (60-90 minutes total drive time)
System groups clients to minimize drive time

Requirements/Qualifications

Valid Physical Therapist license in Oregon
Minimum 1 year of Physical Therapist experience within the last year
Experience in a Home Health setting
Experience with OASIS
BLS (Basic Life Support) certification
