Location: Phoenix, Arizona

Position Details
- Specialty: Registered Nurse (RN) - Wound Care
- Patient Age Group: Pediatrics, Adolescents, Adults, Geriatrics
- Experience Required: Minimum 3 years
- Licensure: Arizona state license or Compact license accepted
- Certifications Required:
- BLS (Basic Life Support)
- Certified Wound Care Specialist
- Weekend Requirement: Every other weekend
- Orientation: Bi-weekly
- First-Timer Friendly: Yes

Required Documentation & Compliance
Identification
- Current and valid Driver’s License, government photo ID, or passport
Background Checks (must be current or completed within specified timeframes)
- Comprehensive background check including SS Trace, 7-year County, National Criminal, and FACIS III
- EPLS/GSA/SAM check (within 30 days of start)
- HHS/OIG exclusion check (within 30 days of start)
- Sex Offender Search (updated annually, within 30 days of start)
- Education Verification (copy of diploma or official transcripts required before start)
- Fingerprinting (if applicable, results on file within 90 days after start)
Certifications (must be current and verified)
- BLS (Basic Life Support) - AHA only
- ACLS (Advanced Cardiac Life Support) - AHA only (if applicable)
- PALS (Pediatric Advanced Life Support) - AHA only (if applicable)
- NRP (Neonatal Resuscitation Procedure) - Essentials or Advanced depending on unit
- TNCC (Trauma Nursing Core Course) - Required for ER, PICU, PCCU units (ATCN accepted in place of TNCC)
- EFM (Electronic Fetal Monitoring) - AWHONN Intermediate (if applicable)
- ENPC (Emergency Nursing Pediatric Course) (if applicable)
- State License Verification - Primary source verification within 30 days prior to start, must be current and unencumbered
- NURSYS Verification (for RN, LPN/LVN, AP) within 30 days prior to start
- Other certifications as required by unit/unit description (e.g., EKG/ECG Interpretation, NIH Stroke Certification, CPI for Behavioral Health)
Facility-Specific Documents
- Contract Staff Orientation Acknowledgment (signed after reviewing orientation booklet)
- Confidentiality Form (signed)
- Monitoring Personnel Job Expectations (signed, not required for behavioral health or non-clinical candidates)
- New Employee Orientation Video completion proof (required before starting assignment)
Health Requirements (time sensitive, many must be completed within 72 hours of signing contract)
- Physical Exam: Must confirm good health, free of communicable disease, no physical limitations
- Fit Mask Test: Completed onsite during orientation
- Immunizations and Titers:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Negative TB test (Qgold or Tspot within 1 year) or Chest X-ray if prior positive
- TB Questionnaire (if positive TB history)
- Rubeola (Measles) - positive titer or 2 vaccines required
- Mumps - positive titer or 2 vaccines required
- Rubella - positive titer or 2 vaccines required
- Hepatitis B - completed vaccine series, positive titer, or declination within last year
- Varicella - positive titer or 2 vaccines required
- Drug Testing: Negative result within 12 months, must include specified drug panels (methamphetamine testing required)
- Seasonal Flu Vaccination or approved medical/religious exemption (required during flu season Oct 1 - Mar 31)
Employment History & Skills
- Current and updated resume covering last 7 years of employment with explanations for gaps over 30 days
- Accurate and updated skills checklist for each position worked in (must be current within 1 year)
Additional Notes
- Candidates can start with some items marked as missing but must complete all required documentation and certifications as specified.
- Behavioral Health Technicians have additional specific requirements not applicable to this RN wound care role.
- Candidates must comply with all facility and regulatory requirements to maintain clearance and continue assignment.
