Experience Requirements

• Must have a valid New York LPN license.
• Hospital experience within the last 2 years is required.
• Must be able to take own patient assignments with RN support.

• SSN Trace, National Criminal, National Sex Offender checks
• Education verification from primary source (hard copies not accepted)
• EPLS/GSA/SAM and HHS/OIG checks within 3 months of start and monthly thereafter
• Foreign equivalency education evaluation if applicable
• Transcript/Diploma if education verification is not on file

Health & Immunization Requirements

Candidates must complete health-related requirements and lab work within 5 days of signing the contract:

• Physical Exam (within 1 year, updated annually)
• Fit Mask test (within 1 year, updated annually; acceptable masks: Halyard duckbill 1870+1860 Regular, 1860 Small; PAPR accepted if failed)
• Negative TB test or Chest X-Ray (within 1 year)
• TB Questionnaire (required in lieu of annual TB test renewal)
• Immunizations or positive titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (Surface AB positive titer, completed series or declination)
  • Varicella
• Drug Testing (within 30 days of start, includes multiple substances)
• Flu Seasonal Vaccination or signed declination (required during flu season)
• Eye Exam (Snellen, within 1 year)
• COVID vaccination copy preferred but not required

Competency Requirements

• Medication Test/Pharmacology Competency Exam (as required by profession)
• Color Blind Testing (required once, not annually)
• Competency Exam (as required by profession/specialty)
