Job Overview

Role Summary

• Inpatient case management role focused on coordinating medical home care, placement, and social support services to support safe discharges.
• Average daily census: 15 cases.
• Floating is required between Bel Air and Aberdeen facilities as needed.

Required Qualifications

• Valid RN license in Maryland
• Associate of Science in Nursing / Associate Degree in Nursing
• Case Management (3 years of experience)
• Case Management experience within the last year
• Experience in an Inpatient setting
• Discharge Planning

Preferred Qualifications

• EPIC charting experience

Certifications

• BLS preferred
