Overview

• Travel assignment in Wisconsin Rapids, Wisconsin
• Home Health Physical Therapist
• Contract length: 13 weeks
• Start date: ASAP
• Guaranteed hours: 38 hours per week, flex up to 40
• Primary location: Wisconsin Rapids
• Shift: 8:00 AM - 4:30 PM
• Schedule: 8 variable days, Monday–Friday
• Weekends: rotates one Saturday every 6–8 weeks
• Holidays: none

Required Qualifications

• AHA or ARC BLS
• Wisconsin licensure
• Valid driver’s license with proof of car insurance
• NPI number
• Bachelor’s Degree in Physical Therapy
• Home Health experience
• Experience working with adult/geriatric patients with orthopedic, neurological, and functional mobility diagnoses
• Knowledge of Physical Therapy standards and practices acquired through a minimum of a Master’s degree in Physical Therapy

Responsibilities

• Evaluates, treats, and plans patient care in accordance with Physical Therapy standards of practice
• Documents to meet regulatory and reimbursement requirements
• Meets productivity expectations of an average of 4.5 visits per day

Work Conditions

• Occasional travel to central and northern regions
• May be low censused and placed on call
• Call-back minimum is 2 hours
• Eligible for 36 hours of unbillable low census hours
• Mileage reimbursed at government approved rate

Additional Notes

• Travelers can be put on call for low census hours, and on-call hours count toward the 36 low census unbillable hours
• If called back, call-back pay applies
• During flu season, anyone declining flu and COVID vaccination must wear a mask at all times; compliance is mandatory
