Location
- Hermantown, Minnesota
Contract Length
- 13 weeks
Schedule
- Monday–Friday, 8:00 AM–5:00 PM
- May be on call nights and weekends
Role Summary
- Provides direct hospice patient care and supportive care to families
- Manages assigned patient clinical records and coordinates care with the interdisciplinary team
- Performs patient assessments, care planning, education, supervisory visits, and follow-up to support quality hospice care
Required Qualifications
- Associate of Science in Nursing / Associate Degree in Nursing
- Valid RN license in Minnesota
- Hospice experience within the last year
- 1 year of hospice experience
- Oncology experience
- Long Term Care experience
- Home Health experience
- BLS issued by American Heart Association
- Driver's License
