Required Qualifications

• Valid RN license in Ohio
• Med Surg experience within the last year
• Associate of Science in Nursing / Associate Degree in Nursing
• Driver's License
• BLS issued by American Heart Association
• Med Surg (1 year of experience)

Onboarding / Compliance Requirements

• TB test within the last 6 months with negative results
• MMR immunity or vaccination documentation
• Varicella immunity or vaccination documentation
• Hepatitis B documentation
• Tdap vaccination within the last 10 years
• Fit test completed within 1 year prior to start
• Physical exam statement within 1 year of start confirming ability to work without restrictions
• Drug screen within 60 days of start
• Criminal background check within 6 months of start
• OIG search within 6 months prior to start
• SAM, EPLS, GSA, and OFAC screening within 6 months prior to start
• I-9 attestation
• Module completion certificate
• Ohio fingerprints or applicable fingerprinting based on residency history

Optional / Conditional Items

• Influenza vaccine proof is required from October through March
• COVID-19 vaccine is preferred, but not required
• If declining flu vaccination, masking attestation and influenza declination forms are required
• If medically exempt from flu vaccination, a medical accommodation form and physician-signed letter are required
• If TB test is positive, a clear chest x-ray and current TB questionnaire are required
• If MMR or Varicella titers are low or non-immune, booster series and repeat titers are required
• If Hepatitis B documentation is not provided as vaccination or positive titers, a signed declination is accepted
• If fingerprint results are pending, an attestation and proof of completion are required
• If background check or fingerprint results contain a hit, a candidate statement is required
• If education was obtained internationally, an equivalency document is required
• If not already included with the background check, Social Security trace and NSO screening may be required
