Overview

• Travel/Contract assignment in Vestal, New York
• 16-week contract
• 8-hour shifts, 5 days per week, evenings
• 300-bed facility
• Ratios: 1:18-20
• First-timers accepted
• Locals accepted
• Every other weekend required upon center request
• Cannot change shift once offered
• RTO only approved at time of submission; cannot be approved after offer
• Cannot take every holiday off
• No float between like units

Required Qualifications

• Valid LPN / LVN license in New York
• Long Term Care (1 year of experience)
• Long Term Care experience within the last year
• BLS issued by American Heart Association or American Red Cross
• Must be able to pass a medication administration (med pass) test and clinical observation
• Strong attendance and punctuality required
• Must be dependable with consistent attendance
• Ability to work in a fast-paced environment
• Must be flexible with scheduling needs
• Resume/Application required
• 2 references required within 2 years of start
• Government issued photo ID required
• Physical exam within 1 year of start
• Negative TB or Chest X-Ray within 1 year of start
• Rubeola (Measles): positive titer or 2 vaccinations
• Mumps: positive titer or 2 vaccinations
• Rubella: positive titer or 2 vaccinations
• Flu vaccine or declination required on file year round
• COVID-19 declination or seasonal vaccination required
• Pneumonia vaccination proof or declination required
• New York NYSIIS Consent Form required
• Medication Test/Pharmacology Competency Exam
• Competency Exam
• Harassment Prevention Training Certificate
• State License 1 Verification
• State License - Copy of Professional License

Preferred

• Supervisors are preferred as references
• Hepatitis B proof, titer, or declination if available
• Varicella is recommended but not required

Additional Details

• Patient population includes Alzheimer’s & Dementia
• Responsibilities include administering medications and treatments, performing medication passes, monitoring and reporting resident condition changes, documenting care, and supervising CNAs
• Attire: any solid color scrubs and closed-toe shoes
• Charting system: PCC not required to submit
• Parking is free
