Job Description

Seeking an experienced LVN/LPN for an outpatient clinic setting. Preference given to candidates with experience in urology or nephrology.

Required Qualifications

• Valid LPN/LVN license in California
• Minimum 2 years of clinic experience, including experience within the last year
• Experience in an outpatient setting
• BLS certification issued by the American Heart Association or American Red Cross
• IV therapy certification

Work Details

• No weekend shifts required
• No floating between departments
• No scheduling requests can be accommodated
• Preference for candidates interested in temp-to-perm opportunities

Compliance and Onboarding Requirements

• Government-issued photo ID required
• Background check including 7-year criminal history, SSN trace, nationwide search, sex offender search, federal district court search, and international criminal searches if applicable
• Education verification of highest level completed
• Physical exam within 1 year confirming ability to perform essential job functions
• Fit mask test within 1 year (updated annually)
• Negative TB test or chest x-ray within 1 year; annual TB questionnaire if history of positive TB
• Required immunizations and titers:

  • MMR (Measles, Mumps, Rubella)
  • Hepatitis B (vaccine series, titer, or declination)
  • Varicella (chickenpox)
• Drug screening panel within 1 year (excluding marijuana/THC)
• Flu vaccine documentation required for clearance during flu season (no declinations accepted)
• Two professional references required, at least one supervisory

Additional Notes

• Fire safety training and certification required within first 6 months of employment
• Medication and competency exams required as per profession standards
• Photo for badge must meet strict requirements (passport-style photo)
• Various facility-specific policies and acknowledgements must be reviewed and signed
