Occupational Therapist - Acute Care Hospital (Travel/Contract)

Location: Lafayette, Indiana
Contract Length: 13 weeks

Job Details

• Work Schedule: Monday to Friday, plus one weekend day per month
• Shift: Day shift, 8:00 AM to 4:30 PM (5 days/week, 8 hours/day)
• Patient Population: Inpatient acute care rehab team treating patients across all acuity levels (ICU to progressive care)
• Common Diagnoses: CABG and other cardiac procedures, stroke, traumatic brain injury (TBI), joint replacement, fracture repairs, respiratory illnesses, dementia, multiple sclerosis (MS), Parkinson’s disease, ambulation issues
• Responsibilities:

  • Evaluate patients and develop/follow plans of care (POC)
  • Provide ADL training, therapeutic exercises, and activities to improve function
  • Assess appropriateness of discharge disposition daily
• Charting System: Cerner (experience preferred)
• Dress Code: Royal blue scrubs, black shoes

Required Qualifications

• Valid Occupational Therapist license in Indiana
• Minimum 1 year of Occupational Therapy experience in an acute care setting within the last year
• NBCOT certification
• Basic Life Support (BLS) certification (AHA or RQI accepted)

Preferred Qualifications

• Experience with Cerner electronic health record system

Compliance & Documentation Requirements

• Valid Driver’s License or Passport (clear copy required)
• Background Check within 90 days of start, including:

  • 7-year residential/work history verification
  • SSN trace, county/state/national criminal checks, sex offender registry, FACIS Level III
  • Education verification (may be pending at start if needed)
• EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
• Official Transcript or Diploma verifying degree and graduation date (may be pending at start)
• State License verification online within 30 days before start
• Medication/Pharmacology competency exam
• Unit competency exam
• Color Blindness testing (Ishihara exam or equivalent, 100% score required)
• Physical exam within 12 months of start
• Fit mask test within 12 months (not required for Physical Therapists; required for OT)
• Tuberculosis screening (PPD, QuantiFERON-TB Gold, or T-SPOT) within 1 year of start
• Immunizations or titers required:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B (series, titer, or signed declination)
  • Tetanus/Diphtheria/Tdap (strongly recommended)
• Negative 10-panel drug screen within 12 months of start
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (9/30 - 3/31)
• References: Minimum 2 from recent employment verified within 2 years; must be from charge, manager, supervisor, or above
• Current employment application with full work history and explanations for gaps over 30 days
• Skills checklist completed within 12 months and annually thereafter

• Prior travel experience is not explicitly required for this position
