Job Opportunity: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) – Home Health

Location: Everett, Washington
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Day shift, 5 days per week (8:00 AM – 4:30 PM)
Hours: 40 hours per week (8 hours per day)

Job Description

• Provide home health nursing care primarily for geriatric patients.
• Average patient load: 5-7 patients per day.
• Work territory may extend up to a 60-mile radius from the office hub.
• Flexibility with travel within the territory is expected.
• On-call duties are required; details to be discussed during the interview.
• Dress code: Scrubs of any color; jeans are not permitted.
• Use of iPad provided; personal cell phones are not allowed during work.
• Mileage reimbursement provided at IRS standard rate.

Required Qualifications

• Valid LPN / LVN license in Washington.
• Minimum 1 year of LPN / LVN experience within the last year.
• Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
• Home health nursing experience.
• Experience with Homecare Homebase (HCHB) Electronic Medical Record (EMR) system.

Compliance and Background Requirements

• Background checks must be current within 90 days of start and include:

  • SSN Trace
  • County searches for the last 7 years
  • FACIS Level 1/MN
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records/Driver’s Record
  • Education verification
  • Work history verification (7 years)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start.
• Fingerprinting required if candidate has not lived in Washington for the past 3 years (completed on-site).
• State-specific criminal history and employability status checks as applicable.
• Signed Acceptable Use Policy required prior to start.
• Agency background attestation must be completed and on file.

Health and Safety Requirements

• Physical exam within the last 12 months.
• Fit test for N95 mask (no specific model required).
• Negative TB test (PPD, Q Gold, or T-Spot) within 1 year or chest X-ray within 5 years if previously positive.
• Annual TB questionnaire required.
• Immunizations or proof of immunity for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Seasonal flu vaccination or facility-specific declination form required.
• COVID-19 vaccination or facility-specific declination required.
• 9-panel drug screen within 12 months of start (instant acceptable).

Additional Requirements

• Two professional references within 2 years of start.
• Complete employment application with 7 years of work history, no gaps.
• Annual skills checklist and competency exams as applicable.
• Medication/pharmacology competency exam required.

• Locals are accepted and compensated at the travel rate.
• Candidates must be flexible with travel territory.
