Physical Therapist - Home Health (Travel/Contract)
Location: San Jose, California
Contract Length: 4 to 6 months (less experienced clinicians must commit to 6 months)
Job Description
- Provide physical therapy services primarily in home health settings.
- Patient focus is on wound care and geriatric home health patients.
- Minimum of 23-24 patient visits per week required to qualify for a 40-hour guarantee.
- No weekend work required.
- On-call is discretionary but not required.
- May be asked to cover surrounding counties; territory details discussed during interview.
- Locals within a 50-mile radius are accepted.
- Charting must be completed onsite using Kinnser/Wellsky systems; no remote charting allowed.
- Dress code: Any color scrubs and closed-toe shoes.
- Free residential parking available.
- COVID vaccination required; declination allowed for religious reasons. Flu vaccination declination allowed with proper documentation.
Required Qualifications
- Valid Physical Therapist license in California.
- Minimum 1 year of Physical Therapist experience within the last year.
- Experience in a Home Health setting.

Compliance & Documentation Requirements
- Government-issued photo ID (Driver’s License or State ID).

- Annual EPLS/GSA/SAM and HHS/OIG checks.
- Certifications: BLS required; ACLS and PALS preferred.
- Physical exam within 6 months signed by MD/DO confirming no active communicable diseases.
- Immunizations or documented exemptions required for:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Measles, Mumps, Rubella (per birth year and documentation)
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, history, or declination)
- Seasonal Flu (vaccination or declination during flu season)
- COVID-19 (vaccination or medical/religious exemption)
- Negative TB test or chest X-ray within 1 year (with specific rules for past positives).
- 9-panel drug screen within 1 year of start.
- Auto insurance card required.
- Emergency contact form required prior to start.
- Professional references: 2 for travel/allied/per diem acute care; 1 for per diem non-acute care (within 3 years).
- Annual skills checklist and competency exams as applicable.
Additional Notes
- Mileage reimbursed from first to last patient visit (not from/to home).
- All required return-to-office (RTO) documents must be submitted with application; no additions accepted after offer.
- Contract length is non-negotiable for less experienced clinicians.
