Physical Therapist - Acute Care Hospital

Location: Mount Vernon, Illinois

Contract Length: 13 weeks

Job Details

• Acute care physical therapy position with 1 year of acute care experience required.
• Rehabilitation hospital experience preferred but not required.
• Shift: Day shift, 2 shifts per week, 10 hours each (7:00 AM - 5:30 PM).
• Workdays include Friday, Saturday, Sunday plus an additional day.
• On-call rotation every 7 weeks for coverage as needed.
• Holiday rotation: one holiday per year.
• Attire: Caribbean blue scrubs or khakis with lab coat.
• Local candidates are accepted.
• Travelers must be clean-shaven to pass mask fit testing; use of personal PAPRs or CAPRs is not allowed.

Required Qualifications

• Valid Physical Therapist license in Illinois (license can be pending at start).
• Minimum 1 year of Physical Therapist experience within the last year.
• Basic Life Support (BLS) certification required.
• Experience with Epic electronic health record system required.

Compliance & Documentation Requirements

• Government-issued photo ID (Driver’s License or State ID).
• Criminal background check within 12 months, including SSN trace, 7-year county and nationwide search, sex offender search.
• Negative drug screen within 12 months of start.
• Fit mask testing to be completed onsite during orientation; must provide fit test record to agency within two weeks of start.
• OSHA Respirator Clearance required prior to start.
• Physical exam within 1 year indicating good health.
• Immunizations and titers per standard healthcare requirements (including Rubeola, Mumps, Rubella, Varicella, Hepatitis B status).
• TB test (PPD, Qgold, or TSPOT) within 12 months; chest x-ray if positive PPD within 5 years.
• Seasonal flu vaccination or approved medical/religious exemption required annually.
• COVID vaccination series preferred.
• Completion of facility-specific orientation modules and documents prior to or during orientation.
• Completion of confidentiality, security agreements, and standards of ethical conduct forms.
• Skills checklist and references as per placement standards.

Additional Notes

• Part-time or full-time considered.
• Candidates must comply with facility mask fit testing policies; failure to shave for fit testing will result in cancellation.
• No personal PAPRs or CAPRs allowed; facility provides necessary respiratory protection.
• Orientation checklists and assessments must be completed and returned within the first week of assignment.
