Required Qualifications

• Valid Physical Therapist license in Montana
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• Current and valid state ID, driver’s license, or passport
• Current legal name must match certifications and licenses
• 2 references, including a direct supervisor reference from within the last year
• Current employment application with 7 years of employment history and explanation for any gaps greater than 30 days
• Must disclose any prior employment at this facility site
• Background check required within 30 days of start and renewed annually
• Education verification of the highest level of education required for the position
• Negative drug screen within 30 days of start and renewed annually
• Negative TB testing required and renewed annually
• COVID-19 vaccination series or approved medical/religious exemption required
• Seasonal flu vaccination or approved exemption required during flu season
• Measles, mumps, rubella, varicella, and hepatitis B immunity/vaccination documentation required
• Tetanus/diphtheria/Tdap documentation preferred
• Physical exam within 1 year of start
• Fit mask testing within 12 months prior to start and renewed annually
• OSHA form required for onsite fit testing
• Must meet minimum education requirements for the position

Certifications and Licensure

• BLS certification required
• State license verification required within 30 days of start
