Required Qualifications

Valid Physical Therapist license in Massachusetts
1 year of Physical Therapist experience
Physical Therapist experience within the last year
Experience in a Home Health setting
1 year of Case Management experience

Schedule

Weekend rotations occur once every 6 weeks

Role Summary

Provide physical therapy services in patients’ homes
Collaborate with patients, families, physicians, and other allied health professionals to coordinate care
Complete electronic documentation and interdisciplinary communications according to agency policies and standards

Onboarding / Compliance

Resume including the past 7 years of work experience and city/state for work and education
2 references within the past 4 years, including at least 1 supervisor or above in the skillset
Skills checklist completed within 1 year of submission
Primary source verification of license and/or certification
Background checks and registry checks required for onboarding
N95 fit test and respirator medical clearance, if required for the skill
Immunization and health documentation required for onboarding
