Required Qualifications

  • Valid Physical Therapist license in Massachusetts
  • 1 year of Physical Therapist experience
  • Physical Therapist experience within the last year
  • Experience in a Home Health setting
  • 1 year of Case Management experience

Schedule

  • Weekend rotations occur once every 6 weeks

Role Summary

  • Provide physical therapy services in patients’ homes
  • Collaborate with patients, families, physicians, and other allied health professionals to coordinate care
  • Complete electronic documentation and interdisciplinary communications according to agency policies and standards

Onboarding / Compliance

  • Resume including the past 7 years of work experience and city/state for work and education
  • 2 references within the past 4 years, including at least 1 supervisor or above in the skillset
  • Skills checklist completed within 1 year of submission
  • Primary source verification of license and/or certification
  • Background checks and registry checks required for onboarding
  • N95 fit test and respirator medical clearance, if required for the skill
  • Immunization and health documentation required for onboarding
