Required Qualifications

• Doctorate of Pharmacy degree
• Valid Pharmacist license in California
• Pharmacist experience within the last year
• 1 year of pharmacist experience

Key Responsibilities

• Review, interpret, and dispense medications according to professional standards and applicable laws and regulations
• Counsel patients on medication use and provide drug information to patients and licensed personnel
• Perform monthly medication regimen reviews and prepare quarterly summaries for skilled nursing facility services
• Attend quarterly quality improvement meetings and provide drug safety education in-services
• Review medication orders and profiles to help prevent adverse drug reactions, duplications, dosing errors, and drug-drug interactions
• Monitor therapeutic treatments and relevant lab values and parameters
• Provide clinical interventions, medication verification, and patient-specific medication safety support
• Compound, pack, label, and dispense medications
• Supervise ancillary staff workflow and verify their work
• Document clinical interventions and monitoring when applicable
• Attend code blue when on duty

Skills and Work Environment

• Knowledge of therapeutic drug monitoring and renal function calculation with dosing adjustment
• Proficiency with automated dispensing units, IV laminar flow hood, and electronic health record systems
• Ability to prioritize work, supervise support personnel, and adapt to changing departmental needs
• Professional communication and teamwork with physicians, nursing staff, departments, and the public
