Required Qualifications
- Doctorate of Pharmacy degree
- Valid Pharmacist license in California
- Pharmacist experience within the last year
- 1 year of pharmacist experience
Key Responsibilities
- Review, interpret, and dispense medications according to professional standards and applicable laws and regulations
- Counsel patients on medication use and provide drug information to patients and licensed personnel
- Perform monthly medication regimen reviews and prepare quarterly summaries for skilled nursing facility services
- Attend quarterly quality improvement meetings and provide drug safety education in-services
- Review medication orders and profiles to help prevent adverse drug reactions, duplications, dosing errors, and drug-drug interactions
- Monitor therapeutic treatments and relevant lab values and parameters
- Provide clinical interventions, medication verification, and patient-specific medication safety support
- Compound, pack, label, and dispense medications
- Supervise ancillary staff workflow and verify their work
- Document clinical interventions and monitoring when applicable
- Attend code blue when on duty
Skills and Work Environment
- Knowledge of therapeutic drug monitoring and renal function calculation with dosing adjustment
- Proficiency with automated dispensing units, IV laminar flow hood, and electronic health record systems
- Ability to prioritize work, supervise support personnel, and adapt to changing departmental needs
- Professional communication and teamwork with physicians, nursing staff, departments, and the public
Compliance and Professional Expectations
- Follow state and federal pharmacy regulations
- Maintain confidentiality, safety awareness, and professional conduct
- Complete continuing education, annual education requirements, annual review, CQI activities, staff meetings, and departmental in-services
