Location: Glendale, California

Job Description

  • Home Health Physical Therapist role.
  • Local candidates are welcome.
  • Home Health experience preferred; if no Home Health experience, outpatient experience is required.
  • OASIS experience is required; candidates without OASIS experience may be considered if they have strong Home Health experience.
  • Hours are based on completed visits: 25 visits equal 40 hours. Hours are guaranteed if 25 or more visits are completed.
  • Mileage reimbursement follows IRS standard.
  • COVID vaccination exemptions are permitted.
  • Candidates may apply with a pending California PT license but must have applied prior to submission.

Required Qualifications

  • Valid Physical Therapist license in California
  • Experience in an Outpatient setting
  • Experience with OASIS

Required Documentation & Compliance

  • Government-issued photo ID (Driver’s License or State ID).
  • Valid passport copy before start.
  • I9 attestation for employment eligibility.
  • Standard background check (7-year search including SS trace, county searches, nationwide sex offender).
  • Annual EPLS/GSA/SAM and HHS/OIG checks.
  • Photo for badge (passport style, shoulders up, front facing, plain background).
  • Personal or auto insurance with current expiration dates.
  • Auto registration documentation with current expiration dates.
  • Signed acknowledgments for training content and hand hygiene policy prior to start.
  • Physical exam within 12 months of start date.
  • Immunizations and health screenings:
    • TDAP vaccine within past 10 years (no declinations accepted).
    • Negative TB test or chest X-ray within 1 year; if past positive TB, annual TBQ and chest X-ray within 5 years required.
    • Measles, Mumps, Rubella (MMR) proof of immunity or vaccination for candidates born in 1957 or later (no declinations accepted).
    • Hepatitis B titer or vaccine series; declination accepted if within 1 year of start.
    • Varicella immunity or vaccination (no declinations accepted).
    • Negative 9-panel drug screen within 1 year (instant drug screen acceptable).
    • Seasonal flu vaccination/documentation required year-round; declinations accepted.
    • COVID vaccination proof or medical/religious exemption required prior to start.

Additional Requirements

  • Two professional references required.
  • Employment application must be completed and submitted for clearance.
  • Annual skills checklist required.
  • Medication/pharmacology competency exam required as applicable (minimum 80% passing score or meet/exceed Angoff score).

  • This is a travel/contract position with a 13-week duration.
