Physical Therapist - Home Health (California, San Jose)

Employment Type: Travel/Contract
Contract Length: 4 to 6 months (less experienced clinicians must do 6 months)


Job Description

  • Provide physical therapy services primarily in home health settings.
  • Patient focus is on quality care rather than number of patients per day.
  • Minimum 23-24 patient visits per week required to qualify for a 40-hour guarantee.
  • Types of cases include Home Health, Wound Care, and Geriatric Home Health patients.
  • Territory may include surrounding counties; details discussed during interview.
  • Locals within a 50-mile radius are accepted.
  • No weekend work required.
  • No on-call requirement (on-call is discretionary but not mandatory).
  • Charting must be completed onsite using provided technology; no remote charting allowed.
  • Dress code: Any color scrubs and closed-toe shoes.
  • Free residential parking available.

Required Qualifications

  • Valid Physical Therapist license in California
  • Minimum 1 year of Physical Therapist experience within the last year
  • Experience in a Home Health setting
  • BLS (Basic Life Support) certification

Compliance & Documentation Requirements

  • Government-issued photo ID (Driver’s License or State ID).
  • Background checks within 1 year, including SS Trace, County searches, and nationwide sex offender registry.
  • Annual verification of exclusion lists (EPLS/GSA/SAM, HHS/OIG).
  • Certifications: BLS required; ACLS and PALS preferred but not mandatory.
  • Physical exam within 6 months prior to start, including clearance of communicable diseases.
  • Immunizations or proof of immunity required for:
    • Measles, Mumps, Rubella (MMR) for candidates born in 1957 or later.
    • Tetanus/Diphtheria/Tdap within 10 years or approved exemption.
    • Hepatitis B (titer, declination, or vaccine series).
    • Varicella (titer, vaccination, history of chickenpox, or declination).
  • Negative TB test or chest X-ray within 1 year of start; if past positive TB, negative chest X-ray required.
  • Drug screening: 9-panel drug screen within 1 year of start (instant drug screen acceptable).
  • Seasonal flu vaccination or facility-specific declination form required during flu season (Oct 1 - Mar 31).
  • COVID vaccination or approved medical/religious exemption required.
  • Auto insurance card required.
  • Two professional references required (travel, allied, or per diem acute care).

Additional Information

  • All required documents must be submitted and approved before starting patient care. Compliance processing may take 2-3 weeks.
  • Requests for time off (RTO) must be submitted at time of application; no additional RTO accepted after offer.
  • Valued clinician qualities: patient care focus, punctuality, self-starter, and timely completion of charting.
