PT Home Health - Travel/Contract Opportunity

Location: Saint Joseph, Missouri

Job Details

Schedule:
- 5 days per week, 8-hour shifts (8:00 AM – 4:30 PM)
- Option for 4x10-hour shifts if preferred
Call: Required 1 Sunday every 5 weeks
Weekend Rotation: Every 5th week, work Saturday and Sunday doing admits, with 2 days off during the week
Coverage Area: 13 counties in Missouri, with travel to more distant areas on a rotating basis; fewer patients scheduled on high mileage/travel days
Dress Code: No printed scrubs; acceptable colors are black, grey, or khaki pants with polo or scrub tops

Required Qualifications

Valid Physical Therapist license in Missouri
Minimum 1 year of Physical Therapist experience within the last year
Experience in a Home Health setting
Basic Life Support (BLS) certification

Compliance & Documentation Requirements

Background checks within 30 days of start, including:
- National criminal check, state and county searches (10 years, all aliases)
- Social Security Number trace with aliases
- National Sex Offender registry
- Federal exclusion lists (EPLS/GSA/SAM, HHS/OIG)
Education verification with official transcript or diploma including graduation date
Missouri EDL Search within 30 days of start
Drug screen within 30 days of start (includes multiple drug panels)
Physical exam within last 12 months (not required annually)
Fit mask testing completed onsite at orientation
Immunizations/vaccinations or documented immunity/declination for:
- Measles (Rubeola), Mumps, Rubella (2 MMR vaccines or positive titer)
- Hepatitis B (vaccination series, titer, or declination)
- Varicella (2 vaccines, positive titer, or declination)
- Seasonal Flu vaccine required during flu season or approved medical/religious exemption (mask required if exempt)
Negative TB test or chest x-ray within 1 year; additional requirements if positive TB history
Completion of facility-specific confidentiality agreement, code of conduct, demographic form, and orientation quiz (minimum 84% score)

Scrubs must be plain (no prints) and in approved colors
Locals accepted at the same rate as travelers
