Overview

• Travel/contract role in Lakeland, Florida
• Day shift: 5x8 hours, 8:00–17:00
• 40 hours per week
• Contract length: 20 weeks
• Locals accepted at the same rate

Required Qualifications

• Valid Physical Therapist license in Florida
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• BLS - Basic Life Support issued by American Heart Association
• Experience in a Home Health setting
• Driver's License
• Experience with equipment and devices (gait belt, rolling walker, sit to stand lift, sliding boards, soft/hard collars, standard walker, supplemental oxygen, upper extremity prosthetics, wheelchair assessment)
• Experience caring for amputee patients
• Experience with wound care

Preferred Qualifications

• OASIS training

Work Setting / Patient Population

• Skilled home care
• Patients may include med/surg, cardiac, ortho, hips/knees/shoulders, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson’s, failure to thrive, respiratory patients, neuro, trachs, trauma, AKA, BKA, and ADLs

Responsibilities / Skills

• Assessment
• Cardiopulmonary/circulatory care
• Documentation
• Equipment and device use, including gait belt, rolling walker, sit-to-stand lift, sliding boards, soft/hard collars, standard walker, supplemental oxygen, upper extremity prosthetics, and wheelchair assessment
• Ortho care
• Neuro care
• Home exercise program
• Medications
• Patient and family education
• Signs and symptoms of physical, sexual, and psychological abuse
• Safety and injury prevention
• Billing

Schedule / Caseload

• Home skilled visits: 6–8 per day
• Minimum of 6 points per day at manager’s discretion, with possible increase by 1 point
• 1–2 weekends per contract

Equipment / Tools

• Laptop
• MiFi for internet access
• Portable printer

Additional Notes

• Any solid color scrub
• Documentation systems used: BayCare Forms, BayCare Scheduling, Ace Timesheet
