Overview

• Travel/contract assignment in Uniontown, Pennsylvania
• Home care / home health setting
• 13-week assignment
• Day shift, Monday–Friday, 8:00 AM–5:00 PM
• Territory covers Uniontown and surrounding area
• Average of 5–6 patients per day
• Weekly productivity expectation: 30 points
• About 50 miles per week of travel
• May be floated to a nearby clinic if patient census is low
• Reason for opening: retirement

Required Qualifications

• Valid Physical Therapist license in Pennsylvania
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• One completed travel assignment
• BLS
• Experience in a Home Health or Inpatient Hospice setting
• Driver's License
• Experience with Homecare Homebase

Schedule

• No weekend rotation
• No on-call

• Home visits and periodic office meetings
• HCHB used for documentation
• Local candidates accepted at the same rate
• Start of Care and Follow Ups are included
• Some western territory positions may use a Baylor shift schedule of 4x10s Friday–Monday
