Shift: 06:30 AM – 03:00 PM
Job Summary
The Registered Nurse provides professional nursing care to a designated group of patients, utilizing the nursing process and collaborating with healthcare team members. Responsibilities include patient assessment, planning, implementation, documentation, evaluation, and emergency response according to established standards of care and nursing core values. The nurse is accountable for ethical, legal, and professional nursing practices and participates in staff development.
Required Qualifications
- Valid Registered Nurse (RN) license in New York State
- Minimum of 1 year of Endoscopy experience, with experience within the last year
- BLS certification issued by American Heart Association or American Red Cross
- ACLS certification
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Valid Driver’s License
Required Onboarding and Compliance Documents
- State license verification completed within 30 days prior to start.
- Centralus Skills Checklist for the assigned unit, updated annually.
- Centralus Core Competency Exam (Nursing) with a minimum score of 80%, updated annually.
- Immunizations and Health Requirements:
- MMR: Positive IgG titer or 2 vaccine series with documented dates.
- Varicella: Positive IgG titer or 2 vaccine series with documented dates.
- Hepatitis B: Positive antibody titer, 3 vaccine series, or approved declination form.
- Tdap: Within the last 10 years or approved declination form.
- TB Test: One of the following within required timeframes: 2 skin PPDs (1 within 12 months, 1 within 3 months), or 1 Quantiferon/Spot test within 12 months. Past positives require chest x-ray within 12 months.
- COVID-19 Vaccine: Optional; proof of vaccine or agency declination.
- Fit Test: Must be completed within the last 12 months prior to start; on-site fit testing available if needed.
- Physical/Health Statement: Within the past 12 months, including clearance with no restrictions, vital signs, height, weight, and vision screening.
- Drug Screen: 9+ panel excluding marijuana (not permitted to be tested in New York).
- Background Checks:
- Agency background check within 30 days of start, including SSN address trace, county and national criminal searches, FACIS 3 check.
- 7 Instant Background Checks (manual upload required).
- Identification: Valid US driver’s license or state ID (not expired).
- Professional References: Two references required (one supervisor from last 12 months, one supervisor or peer from last 3 years).
- Employment Verification: Verification of most recent employer (agency counts as employer).
- Resume: Complete employment and education history for last 7 years.
- Signed Job Description and other required agency documents (Code of Conduct, Sexual Harassment Acknowledgement, Confirmation of Assignment, Self Study Packet and Quiz, Parking Registration Form).
- Centralus Epic Questionnaire and TB Questionnaire Form (specific to facility).
- Education Verification: Initial nursing degree verification recommended but optional for onboarding.
- Education Mission Statement: Must be reviewed and signed for initial assignments.
- Centralus Color Headshot: Recent photo with solid background, no filters or sunglasses.

- The nurse will report to the assigned Unit Director.
- Provides age-appropriate care to all patients.
- Expected to maintain awareness of current nursing research and apply findings as appropriate.
