Job Description

• Work with Philips G3 Sleepware system
• Provide polysomnography services during overnight shift

Required Qualifications

• Minimum of 1 year of experience as a Polysomnographer, including experience within the last year
• BRPT (Board of Registered Polysomnographic Technologists) certification
• BLSEP (Basic Life Support for the Polysomnographic Technologist) certification

Immunizations & Health Screenings

Background & Verification Checks

• Federal, nationwide, and county criminal background check (last 7 years) including SSN verification
• Iowa-specific criminal history, sex offender, dependent adult & child abuse registry checks (within 30 days prior to start)
• Education verification of highest academic degree or diploma/GED
• EPLS (Excluded Parties List System) check within 30 days
• OIG (Office of Inspector General) exclusion check within 30 days
• NSO (National Sex Offender) check within 30 days
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days

Documentation & Agreements

• Resume and skills checklist (within 1 year)
• 2 professional references
• Signed confidentiality, conflict of interest, code of conduct, integrity and audit services forms
• Specialty-specific competency testing and CORE exam within 1 year
• System access form completed and uploaded one week before start
