Overview

• Provides physical therapy evaluation, treatment, and care planning for patients across pediatric, adolescent, geriatric, and general populations.
• Works in a therapy department serving mostly outpatient patients, with some inpatient/swing bed care.
• May supervise physical therapy support staff.

Required Qualifications

• Valid Physical Therapist license in Illinois.
• BLS.
• Graduate degree from an approved school of physical therapy or certificate in physical therapy from an accredited college or university.

Key Responsibilities

• Evaluate patients and develop, direct, and administer physical therapy treatment plans.
• Observe treatment effects and recommend changes to the physician as needed.
• Perform patient assessments and provide teaching to patients and families.
• Maintain accurate documentation, records, and patient charges.
• Follow infection control, safety, quality, and fire safety procedures.
• Operate equipment safely and maintain department cleanliness and safety.
