Location: Salem, Massachusetts
Employment Type: Long-Term Assignment (13 weeks)
Shift: Monday through Friday, 8:00 AM to 4:30 PM (32 hours, Days)
Unit: Case Management – Inpatient medical/surgical patients

Job Description

• Acute Care RN Case Manager role focused on inpatient medical/surgical patients.
• Coverage includes utilization management and discharge planning.
• Clinician to patient ratio approximately 1:15.

Required Qualifications

• Valid RN license in Massachusetts.
• Minimum 3 years of case management experience, including acute hospital case management.
• Case management experience within the last year.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Experience in utilization management.
• Experience in discharge planning.

Required Credentials & Compliance

• Primary source verification of RN license and certifications within 30 days of start date; must be maintained and updated upon expiration.
• BLS certification must be from AHA (only AHA accepted for this location).
• N95 Fit Test within 365 days of start date; must be completed prior to assignment (no onsite testing).
• Respirator Medical Clearance attestation required if fit test is needed.
• TB screening: documentation of negative TB skin test or blood test within 3 months of start date; updated if break >6 months.
• Health Statement signed by MD/DO/NP/PA within 365 days of start date; updated if break >6 months.
• MMR vaccination or proof of immunity required.
• Documentation of TB symptom survey if positive PPD.
• Criminal background check within 45 days prior to start; includes Massachusetts CORI, SSN trace, national sex offender check, and exclusion checks.
• Medication exam with 80% passing score within 365 days.
• Education verification of highest level achieved.
• Submission of 7 years of work history including city and state.
• Two professional references within 4 years, including at least one supervisor-level reference.
• Completion of skills checklist within 1 year.
• Confirmation of assignment form required upon acceptance and for any start date changes.

Additional Information

• Strongly preferred (but optional): Hepatitis B vaccination or proof of immunity.
• Tdap vaccination within 10 years preferred.
• Positive PPD requires negative chest x-ray documentation.
• Name change documentation required if legal name differs from credential documents.
