Assignment Length: 8 weeks
Shift: Day shift, 4x10 hours (06:45 AM – 5:15 PM)

Job Description

• Operating Room Registered Nurse position
• 4 ten-hour day shifts per week
• Local candidates will not be considered

Required Qualifications

• Valid RN license in New Hampshire
• Minimum of 1 year of Operating Room (OR) experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)

Health and Compliance Requirements

• Documentation of immunity or vaccination for:

  • Measles, Mumps, Rubella (two MMR vaccines or proof of immunity via blood test)
• Tuberculosis (TB) screening:

  • TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date
  • If positive TB test, documentation of a negative chest x-ray after positive test required
• Health statement signed by MD/DO/NP/PA within 1 year of start date
• N95 Respirator Fit Test within 1 year of start date, updated annually (no exemptions allowed)
• Respirator Medical Clearance attestation required if fit test is required

Background and Credentialing Requirements

• Criminal background check within 45 days prior to start date, including county and international residence/travel history for last 7 years
• New Hampshire state-specific criminal background and BAAS checks within 30 days prior to start date
• National sex offender registry check within 30 days prior to start date, updated annually
• Exclusion checks (OIG, GSA, Medicaid lists) within 30 days prior to start date, updated annually
• Social Security Number trace for last 7 years addresses and aliases
• Verification of highest level of education achieved (HS/AS/BA/MA/advanced degree or certification)
• Resume including past 7 years of work experience with city and state
• Two professional references within 4 years, including at least one supervisor or higher
• Skills checklist completed within 1 year of submission
• Confirmation of assignment/POI form required upon acceptance and for extensions

Additional Notes

• Fit testing cannot be performed onsite; must be completed prior to assignment start
• Emergency NH license applicants should apply for emergency license within 48 hours of permanent license application
• Tdap vaccine within 10 years preferred but not required
• Hepatitis B vaccination or immunity strongly preferred but optional
• Name change documentation required if legal name differs from credential documentation
