Job Description
- Perform screening and diagnostic mammography
Required Credentials and Documentation
Submission Requirements (Before Assignment)
- Current professional resume/profile
- Valid driver's license or state ID (must not be expired and must match address on file)
- License/Certificate verification with primary source verification within 30 days prior to start
- At least two professional references
- Ascension Resume
Onboarding Requirements (Must be completed and uploaded prior to or at start)
- Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, completed within a year, updated yearly)
- Medical Imaging Contract Worker Onboarding Education Attestation (6 hours pre-hire training with signed attestation)
- Ascension Badge Photo (forward-facing headshot with white or blue background, no alterations)
- Health and Immunization Documentation:
- MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
- Tdap: Valid within 10 years (no declinations accepted)
- Hepatitis B: Vaccination series completion or signed declination
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
- Fit Test: Required for all travelers; preferred before start, can be done onsite
- 2 Step PPD or IGRA (Quantiferon/Tspot): Recent testing required with annual TB questionnaire
- Positive PPD: Negative chest X-ray within one year and annual TB questionnaire (optional but recommended)
- Statement of Good Health or completed physical exam prior to first assignment, updated annually
- Ascension Criminal Background Check (including SSN trace, employment verification, OIG search, national sex offender check; updated annually)
- EPLS (Excluded Parties List System) check within 90 days of start
- Ascension Skills Checklist (clinical specialty-specific, completed within one year, updated annually)
- 12 Panel Drug Screen within 30 days prior to start, updated annually
- Education Verification relevant to clinical work within 90 days of start
- Employment Verification for past 7 years, updated annually
- Confirmation of Assignment on agency letterhead with start/end dates
Additional Notes
- Some forms and onboarding steps are completed onsite and uploaded by the facility.
- Declinations are not accepted for MMR, Varicella, and Tdap vaccines.
- Flu vaccine declinations require mask-wearing during flu season.
- Background checks and verifications must be current and updated if there is a break in service longer than 90 days.

In the mammogram department the managers are kind. The work load is low to steady. The staff is drama free. The schedule/location rarely changes. All and all great place to work.
Cons
Lunches are not always blocked off, only given 45 minutes. So might have to bring ur lunch. And eat at your work station.
