Valid Speech Language Pathologist license in Texas
Experience working with Pediatric patients
Feeding experience
AAC (Augmentative and Alternative Communication) experience
ASHA certification required for SLP
Unexpired government-issued photo ID
Background check within 1 year of start, including SS Trace and county searches for the last 7 years of locations lived/worked/in school, nationwide sex offender search, updated annually
EPLS/GSA/SAM clearance prior to facility start
HHS/OIG clearance prior to facility start
Orientation Safety Manual acknowledgment, updated annually
State license verification prior to facility start and upon expiration, as applicable by profession
Physical exam statement within the last 12 months of start
Negative TB or chest X-ray within the required timeframe
TB questionnaire annually for positive TB and in lieu of annual TB test
Rubeola documentation
Mumps documentation
Rubella documentation
Hepatitis B documentation
Varicella documentation
9-panel drug screen within the last 12 months of start
2 professional references within 3 years of hire
