Overview

• Travel/contract role in Fond du Lac, Wisconsin
• Outpatient setting
• Start: ASAP
• Daily caseload: 10–12 patients
• Shift options: 4x10s or 5x8s
• Hours: 40 per week

Required Qualifications

• Valid Physical Therapist license in Wisconsin
• 1 year of Physical Therapist experience
• Physical Therapist experience within the last year
• BLS issued by American Heart Association
• Strong orthopedic skills

Schedule and Work Expectations

• No weekend requirement
• No on-call requirement
• No floating outside scope
• Local candidates are not accepted; local radius is 70 miles
• One RTO request per contract
• First-time travelers are accepted

Clinical Focus

• Adult outpatient orthopedics, including TKA, THA, joint pain, and back pain
• Adult neuro cases, including post-CVA and Parkinson’s

Compliance and Onboarding

• Must shave for mask fit testing
• OSHA respirator clearance required before start
• Fit testing is completed onsite during the first week of orientation
• Skills checklist required
• Wisconsin caregiver screening required before start
• Wisconsin BID form required
• Background check required within 12 months of start
• Drug screen required within 12 months of start
• TB screening required within 12 months of start
• Photo for badge is required for all candidates except at this facility
• Confidentiality and security agreement must be signed before start
• Standards of ethical conduct and relevant policies must be reviewed and signed before start
