Patient Care Manager for a Home Health office
Manages dashboard review, orders, physician and clinician contact, new admissions, and may assist with scheduling
May need to assist with visits outside the office
Floating is within facility/scope of practice as needed
Call may be required, possibly 1 per week
Weekends as needed
Maximum requested time off: 7 days

HCHB experience needed until 10/1/2026
Epic experience needed after 10/1/2026
If no HCHB experience, completion of an online HCHB course is required before start, at traveler upfront cost
