Overview

Travel/Contract role in Lafayette, Indiana
Day shift: 3x12 hours, 6:00–18:30
12-week assignment
First-time travelers accepted
Every other weekend required
Floating required within scope of practice
Locals not accepted
Must work 2 out of 3 major holidays

Required Qualifications

Valid RN license in Indiana
Post Partum experience within the last year
1 year of Post Partum experience
BLS issued by American Heart Association
Associate of Science in Nursing / Associate Degree in Nursing
ACLS - Advanced Cardiac Life Support
PALS - Pediatric Advanced Life Support
NRP - Neonatal Resuscitation Procedure
State license verification
Unit competency exam
Medication Test/Pharmacology Competency Exam
Color Blind Testing Online
NIH Stroke Certification
Minimum of 2 references from recent employment
Current employment application with all gaps of 30 days or more explained
Skills Checklist 1 completed within 12 months of start and annually
Application history covering the past 7 years with all gaps accounted for
Physical exam completed within 12 months of start
Fit mask test within 12 months of start
Negative TB test, QuantiFERON-TB Gold, or T-SPOT within 1 year of start
TB questionnaire
Rubeola (Measles): positive titer or 2 vaccines
Mumps: positive titer or 2 vaccines
Rubella: positive titer or 2 vaccines
Hepatitis B series, titer, or signed declination
Varicella: positive titer or 2 vaccines
