Job Description

• 59-day assignment (approximately 8 weeks)
• Work involves Radiology/Cardiology specialty with focus on CT (Computed Tomography)
• Proficient in EMR, RIS, HIS, PACS, and modality-specific software

Required Qualifications

• High School Diploma or GED
• Graduate of an accredited Radiography program
• Current New York State license in Radiography
• Certification by New York State to inject contrast media
• American Registry of Radiologic Technologists (ARRT) certification in Radiography and Computed Tomography
• Maintain continuing education credits per ARRT requirements
• Minimum 1 year of experience in Diagnostic Imaging (higher experience requirement applies)
• Recent relevant experience in CT preferred

Certifications

• ACLS (Advanced Cardiac Life Support) – AHA Primary Source Verification required
• BLS (Basic Life Support) – AHA Primary Source Verification required
• PALS (Pediatric Advanced Life Support) – AHA Primary Source Verification required
• State License Verification – must be current and active with no pending adverse action
• Tech Certification Verification – must be current/active with no pending adverse action

Background and Compliance Requirements

• Criminal background check within 60 days prior to assignment covering all locations lived, worked, or attended school in the past 7 years, including all aliases
• EPLS/GSA/SAM, HHS/OIG, Social Security Verification, Sex Offender Search, OFAC Sanctions List Search within 60 days prior to start
• Education Verification with primary source verification for all formal education listed
• 20-year Employment Application and Work History Verification (including all gaps accounted for)
• 2 professional references within 2 years of start date

Health and Safety Requirements

• Physical exam within 12 months of start including vital signs, review of systems, medical history, and clearance to work without restrictions
• Fit mask test (N95) within 1 year prior to start (3M 1870+ or Halyard N95 Small/Regular)
• Negative TB test or chest X-ray per specified options within required timeframes
• Annual TB Questionnaire completion
• Vaccination or immunity documentation for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B (antibody titer, antigen titer, or vaccine series with declination if applicable)
  • Hepatitis C screening within 6 months of start
  • Seasonal Flu vaccination during flu season (Sept 3 – May 4), with medical or religious exemptions requiring approval
• Negative 9-panel drug screen with chain of custody within 60 days prior to start (excluding marijuana)
• Vision screening (Snellen) within 12 months of start; corrective lenses noted if applicable
• Color blind testing if applicable (after two failed online attempts)

Competency Requirements

• Medication Test/Pharmacology Competency Exam as applicable
• Competency Exam as applicable to specialty

Facility-Specific Requirements

• Signed Confidentiality Agreement and Staff Attestation prior to start
• Completion of Department Orientation Checklist onsite
• Completion of required prestart and annual training modules
• Acknowledgment of Orientation Safety Manual (updated annually)
• Completion of FBI Wanted Person List and Terrorist Watch List checks within 60 days prior to start and updated annually
• Compliance with New York State Central Registry and Statewide Exclusion List forms and results as applicable
