Position Details
- Specialty: Registered Nurse (RN) - Wound Care
- Patient Age Group: Pediatrics, Adolescents, Adults, Geriatrics
- Experience Required: Minimum 3 years
- Licensure: Arizona state license or Compact license
- Certifications Required:
- BLS (Basic Life Support)
- Certified Wound Care Specialist
- Weekend Requirement: Every other weekend
- Orientation: Bi-weekly
- First-Timer and Local Candidates: Accepted
Required Documentation & Compliance
Identification
- Current and valid Driver’s License, government photo ID, or passport
Background Checks (must be current or completed within required timeframes)
- Criminal background check (including SS Trace, 7-year County, National Criminal, FACIS III) within 1 year
- EPLS/GSA/SAM check within 30 days of start
- HHS/OIG exclusion check within 30 days of start
- Sex Offender Search within 30 days of start (annual update required)
- Education Verification (copy of diploma or official transcripts accepted)
- Fingerprinting (if applicable to role)
Certifications (AHA preferred where applicable)
- BLS (Basic Life Support)
- ACLS (Advanced Cardiac Life Support)
- PALS (Pediatric Advanced Life Support)
- NRP (Neonatal Resuscitation Procedure)
- TNCC (Trauma Nursing Core Course) or ATCN for ER, PICU, PCCU
- EFM (Electronic Fetal Monitoring) - AWHONN Intermediate
- ENPC (Emergency Nursing Pediatric Course)
- EKG/ECG Interpretation Certification (if applicable)
- NIH Stroke Certification (if applicable)
- CPI (Crisis Prevention Institute) for Behavioral Health roles
Licensure Verification
- Primary source verification of state license within 30 days prior to start (must be current and unencumbered)
- NURSYS verification within 30 days prior to start
Health Requirements (time sensitive, mostly within 72 hours of contract signing)
- Physical exam confirming good health and ability to perform job duties (valid within 1 year)
- Fit mask test (completed onsite during orientation)
- Immunizations and Titers:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Negative TB test (Qgold or Tspot within 1 year) or Chest X-ray if prior positive
- TB Questionnaire if positive TB history (within 30 days)
- Rubeola (Measles) - positive titer or 2 vaccines required
- Mumps - positive titer or 2 vaccines required
- Rubella - positive titer or 2 vaccines required
- Hepatitis B - vaccination series, positive titer, or declination within last year
- Varicella - positive titer or 2 vaccines required
- Drug Testing: Negative result within 12 months (specific panels required)
- Seasonal Flu Vaccination or approved medical/religious exemption (required during flu season Oct 1 - Mar 31)
Facility-Specific Requirements
- Completion of contract staff orientation acknowledgment and confidentiality forms
- Viewing of New Employee Orientation (NEO) video and submission of proof of completion
- Job summary and skills validation completed by facility after assignment start (not required for all candidates)
Employment History & References
- Updated resume including last 7 years of employment with explanations for gaps over 30 days
- Current and accurate skills checklist (within 1 year)
- References (Aya requirement, not facility requirement)
Additional Notes
- Candidates may start with some items marked as missing but must complete them as required.
- Behavioral Health Technicians and certain other roles may have additional specific requirements.
- Candidates must comply with all documentation and health requirements to maintain clearance and continue assignment.
