Health & Immunization Requirements

- Fit Mask: Required within 1 year, updated annually. Acceptable masks include Halyard duckbill 1870+, 1860 Regular, 1860 Small. If failed, PAPR use onsite allowed with proof.
- Negative TB or Chest X-Ray: Negative Qgold, Tspot, or 2-step PPD within 1 year. For positives, negative chest X-ray within 10 years and annual TB questionnaire required.
- TB Questionnaire: Required in lieu of annual TB test renewal.
- Rubeola (Measles): Positive titer or 2 vaccines required; boosters may be accepted. Age exemption if born before 1957.
- Mumps: Positive titer or 2 vaccines required; boosters may be accepted. Age exemption if born before 1957.
- Rubella: Positive titer or 1 MMR/Rubella vaccine required; boosters may be accepted. Age exemption if born before 1957.
- Hepatitis B: Positive titer, completed series, or declination required. Dialysis candidates require additional titers.
- Varicella: Positive titer, 2 vaccines, or documented history required; boosters or declination accepted.
- Drug Testing: Required within 30 days of start, includes multiple substances. Not required annually.
- Flu Vaccination/Documentation: Required during flu season or signed declination.
