Negative TB or Chest X-Ray: Negative Qgold, Tspot, or 2-step PPD within 1 year. For positives, negative chest X-ray within 10 years and annual TB questionnaire required.
TB Questionnaire: Required in lieu of annual TB test renewal.
Rubeola (Measles): Positive titer or 2 vaccines required; boosters may be accepted. Age exemption if born before 1957.
Mumps: Positive titer or 2 vaccines required; boosters may be accepted. Age exemption if born before 1957.
Rubella: Positive titer or 1 MMR/Rubella vaccine required; boosters may be accepted. Age exemption if born before 1957.
