Location: Bemidji, Minnesota
Contract Length: 13 weeks

Job Details

Facility Type: Long Term Care (LTC) and Skilled Nursing Facility (SNF)
Number of Beds: 78
Patient Population: General LTC population and some short-term care
Nurse-to-Patient Ratios:
- Day/Evening shifts: 1:20
- Night shifts: 1:40
Electronic Medical Record (EMR) System: Matrix
Shifts: Both 8-hour and 12-hour shifts available
Weekend Requirement: At least every other weekend
Holiday Requirement: Half holidays worked
Location-specific orientation provided
No on-call or call-back required or billable

Required Qualifications

Valid LPN / LVN license in Minnesota
Minimum of 1 year of Long Term Care experience within the last year
BLS (Basic Life Support) Certification

Additional Notes

Backouts or quits with less than 14 days’ notice will incur a $500 fee
Local rates apply within 80 miles or if using provided housing
No charge differential at certain facilities
