Overview

Interim Director of Care Management
Location: Fort Wayne, Indiana
Contract length: 13 weeks
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Call: After-hours call may be needed; leadership accountability is expected 24/7
Orientation dates: 9/21, 10/5, 10/19, 11/2
Scrub color: Any

Required Qualifications

Valid RN license in Indiana
Bachelor of Science in Nursing
Case Management (3 years of experience)
Case Management experience within the last year
Experience in an acute care setting
Experience in director roles
Acute hospital case management experience
Discharge planning experience

Role Summary

Oversees implementation, development, and management of care management and resource management programs
Ensures compliance with regulatory and accreditation standards
Leads care management staff and supports discharge planning and patient-centered care
