Adjust care plans as necessary based on patient response. </li> <li>Medication Administration: Administer medications as prescribed, monitor for side effects, and ensure proper dosage and timely delivery. Assist with pain management strategies to promote patient comfort during rehabilitation</li> <li>Patient Education: Educate patients and families about recovery goals, rehabilitation techniques, safety precautions, and strategies to promote independence in daily activities</li> <li>Emergency Response: Respond to medical emergencies, including sudden changes in condition, respiratory distress, or cardiac issues, and act quickly to stabilize patients when needed.</li> <li>Documentation: Maintain accurate and up-to-date patient records, documenting assessments, treatments, and progress toward rehabilitation goals in the electronic health record (EHR)</li> <li>Discharge Planning: Assist with discharge planning, ensuring patients and families are prepared for postdischarge care, including home modifications, continued therapies, and follow-up appointments</li> <li>Collaboration: Work closely with interdisciplinary teams, including physicians, physical therapists, occupational therapists, case managers, and social workers, to provide comprehensive care tailored to each patient's needs</li></ul>The ideal candidate must have at least one year of experience as an RN in a rehabilitation setting. Current BLS certification is required. Additional certifications may be required.
