Valid Speech Language Pathologist license in Kentucky
Speech Language Pathologist (3 years of experience)
Speech Language Pathologist experience within the last year
BLS - Basic Life Support issued by American Heart Association
CCC-SLP
Experience in a(n) Acute Care setting

Flu vaccination required; no exemptions
Valid driver’s license, state ID, or passport required
Background check required
Kentucky Vulnerable Adult Maltreatment Registry search required
2 clinical references required
7-year work history required
Complete application required

Health Requirements

Physical exam within 1 year
Fit mask test within 1 year
Negative TB or chest X-ray required
Immunity or documentation required for measles, mumps, rubella, hepatitis B, and varicella
Negative 13-panel drug screen within 12 months of start
COVID-19 vaccination, exemption, or declination required for reporting purposes

Additional Notes

Patient age groups include adolescents, adults, and geriatrics
