Valid Speech Language Pathologist license in California
Speech Language Pathologist (1 year of experience)
Speech Language Pathologist experience within the last year
Minimum of 2 references covering 1 year of employment total within the last 3 years in a comparable specialty/setting
Employment application must include at least the last 5 years of employment
Skills checklist required for the applicable unit
Physical exam within 1 year of start, signed by MD, NP, PA, or DO
Negative TB or chest X-ray documentation within 12 months of start
Drug screen within 12 months of start
Tdap within 10 years, or declination
Rubeola, mumps, rubella, hepatitis B, and varicella documentation or declination as specified in the source
COVID vaccination record or declination is expected per source guidance
Fit mask clearance required for travel starts
RespSafety clearance required for travel starts
Background check and education verification required
EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
State license verification required as applicable
BLS - Basic Life Support
ACLS - Advanced Cardiac Life Support
PALS - Pediatric Advanced Life Support
NRP - Neonatal Resuscitation Procedure
