Care Coordinator (Registered Nurse) - Travel/Contract

Location: Paoli, Pennsylvania
Contract Length: 13 weeks

Job Summary

• Role: Care Coordinator in Social Services
• Facility Type: Acute Care, Level II Teaching Hospital
• Total Staffed Beds: 261
• Shift: Day shift, 5 days a week (8:00 AM - 4:30 PM)
• Hours: 40 hours per week, 8 hours per day
• Patient Ratios: 15-20 patients per nurse
• Weekend Requirement: No mandatory weekends; optional weekend work available
• On-call: Not required
• Charting System: Epic
• Scrub Color: Wine-colored scrubs preferred but not mandatory

Required Qualifications

• Valid Registered Nurse (RN) license in Pennsylvania
• Minimum of 1 year of case management experience, including within the last year
• BLS certification issued by the American Heart Association or American Red Cross
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience in acute care settings

Additional Requirements

• Background Checks:

  • Criminal background check within 6 months covering all locations lived/worked/schooled in past 7 years
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
• Education Verification: Diploma, degree, or transcripts required
• Health Documents:

  • Physical exam or statement of good health within 1 year prior to assignment, updated annually
  • Fit mask test within 1 year, updated annually (specific approved masks required)
  • Negative TB test (QuantiFERON Gold or T-Spot) within 6 months prior to assignment
  • Immunizations: MMR (Measles, Mumps, Rubella), Varicella, Hepatitis B, Tdap vaccine
  • Drug screening: 10-panel drug screen within 45 days of start, updated annually
  • Flu vaccine or approved exemption during flu season (Sept-April 1)
  • Respirator clearance within 1 year, updated annually

• Proof of valid government-issued photo ID and driver's license without restrictions required

• Return to Office (RTO) restrictions are at manager's discretion
