Required Qualifications

Valid RN license in Utah
Hospice (3 years of experience)
Hospice experience within the last year
BLS
Associate of Science in Nursing / Associate Degree in Nursing
Driver's License

Role Overview

Provide skilled nursing care to terminally ill patients and support their families in accordance with the plan of care and hospice philosophy
Perform comprehensive patient assessments and coordinate individualized care plans
Manage pain and symptoms
Educate patients and families about disease process, self-care, end-of-life issues, and hospice philosophy
Collaborate with an interdisciplinary team
Participate in on-call rotation

Preferred Experience

Hospice, home health, or palliative care experience

Additional Details

On-call coverage includes 1 to 3 shifts per week, including 3 weekends during the contract
Mileage reimbursement is provided for home visits
