Hospice RN

Required Qualifications

  • Valid RN license in Utah
  • Hospice (3 years of experience)
  • Hospice experience within the last year
  • BLS
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Driver's License

Role Overview

  • Provide skilled nursing care to terminally ill patients and support their families in accordance with the plan of care and hospice philosophy
  • Perform comprehensive patient assessments and coordinate individualized care plans
  • Manage pain and symptoms
  • Educate patients and families about disease process, self-care, end-of-life issues, and hospice philosophy
  • Collaborate with an interdisciplinary team
  • Participate in on-call rotation

Preferred Experience

  • Hospice, home health, or palliative care experience

Additional Details

  • On-call coverage includes 1 to 3 shifts per week, including 3 weekends during the contract
  • Mileage reimbursement is provided for home visits
