Hospice nursing position focused on providing care in patients’ homes and other residential settings
Weekday schedule: Monday–Friday, 8:00 AM–5:00 PM, with occasional weekend and on-call coverage
40 hours per week
Contract length: 12 weeks

Provides compassionate hospice care to patients and families
Educates patients and caregivers about goals of care and home care support
Manages an assigned caseload as the primary nurse
Leads the interdisciplinary care team for assigned patients
Works with patients across a range of living settings, including private homes, nursing homes, group homes, and assisted living facilities
