Registered Nurse Case Manager / Primary Care Clinic Nurse

Location: Juneau, Alaska
Employment Type: Contract (13 weeks)
Shift: Monday to Friday, Days

Job Overview

This is a hybrid role combining case management and clinic nursing responsibilities. The nurse will actively manage assigned patients and serve as a clinic nurse, collaborating with medical providers and interdisciplinary teams to optimize patient outcomes.


Key Responsibilities

  • Manage acute, chronic, and health maintenance needs of patient panels through EHR review, outreach, and education.
  • Coordinate multi-disciplinary care conferences for high-risk or complex patients.
  • Participate in case management reviews with multi-disciplinary teams.
  • Advocate for patients and families, promoting decision-making, independence, and self-care.
  • Facilitate and track patient referrals within and outside the organization, ensuring timely follow-up and resolution of obstacles.
  • Perform clinic nursing duties including assessment, documentation, education, and medication/therapy administration.
  • Maintain a clean, safe work environment and adhere to infection control policies.
  • Participate in triage and respond to emergency situations as an RN.
  • Collaborate effectively with team members across departments and contribute to continuous improvement efforts.

Required Qualifications

  • Current, full, unrestricted Registered Nurse license in Alaska at time of application.
  • Graduate of an accredited nursing school (Bachelor’s degree preferred).
  • Basic Life Support (BLS) certification.
  • At least 1 year of case management experience within the last 2 years.
  • Proof of flu vaccination required.

Skills and Knowledge

  • Knowledge of nursing process and ability to apply it in clinical settings.
  • Familiarity with customer service, safety, and infection control principles.
  • Proficient in assessment, data collection, and use of medical and computer equipment.
  • Strong oral and written communication skills.
  • Ability to multi-task, work independently, and maintain compliance with annual competencies.
  • Problem-solving and conflict resolution skills to foster positive working relationships.
  • Ability to work in a fast-paced clinical environment, including physical demands such as standing for long periods, bending, stooping, and moving quickly.
  • Ability to follow complex directions and perform multiple tasks simultaneously.

Additional Information

  • Candidates must live at least 50 miles away from the work location to be eligible.
  • No housing or travel accommodations provided.
  • Start date will be at least 6 weeks after fingerprint cards are received.

This position requires active Alaska RN licensure and recent case management experience. It offers a full-time, weekday day shift schedule in a hybrid clinical and case management role.
