Description
American Traveler is hiring an experienced RN Case Manager for a hybrid primary care/clinic nurse role in Juneau, AK, requiring an active Alaska RN license and recent case management experience.
Details
- Clinic/ambulatory care setting with a hybrid role combining active case management and staff nursing duties
- Manages both acute and chronic care needs for a primary care patient population, including high-risk and complex cases
- Coordinates multidisciplinary care conferences and referral processes for assigned patients
- Reviews EHR reminders and triggers to provide patient outreach and education
- Monday through Friday, day shift schedule
- 13-week contract based in Juneau, AK
Requirements
- Active, unrestricted Alaska RN license required at time of consideration
- Current BLS certification required
- Minimum 1 year of case management experience, with experience must be within the last 2 years
- Graduate of an accredited nursing school required; Bachelor's degree preferred
- National Case Management certification preferred
- Ability to work at a quick pace, stand for long periods, bend, stoop, and move rapidly in a fast-paced clinical environment
- Proof of flu vaccine required
- Fingerprint cards must be submitted as part of the onboarding process
Additional Information
- Serves as both a case manager and clinic staff nurse, performing assessments, documentation, patient education, and administering medications and therapies
- Functions as a patient advocate at the service-delivery level, supporting patient and family decision-making and independence
- Coordinates and tracks patient referrals within and outside the organization, proactively identifying and resolving delays in care
- Participates in triage and responds to emergency situations as part of the clinical team
- Collaborates with providers and interdisciplinary teams to establish individualized patient goals and improve outcomes
- Candidates must reside at least 50 miles from the facility to be eligible; housing and travel are not provided by the facility
- Note that the start date is a minimum of 6 weeks from the date fingerprint cards are received by the facility
