- Background check: 7-year criminal history, sex offender, FACIS Level 3, education verification, and international searches as applicable
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start and updated for renewals
- Education verification through primary source (transcripts/diplomas not accepted as sole proof)
- State nursing license verification (Washington D.C. license only, must be current and valid)
- NURSYS verification for all RNs
- Drug screening: 9-panel drug screen within last 12 months (instant acceptable)
- Physical exam: Initial and updated annually, must meet physical demands of position
- Fit mask test within one year of start, updated annually
- Immunizations and titers required:
- Tetanus/Diphtheria/Tdap (vaccine or titer; declination accepted with form)
- Negative TB test or chest X-ray within 1 year
- Rubeola (Measles), Mumps, Rubella (titer or 2 vaccines; booster if low titer; declinations only for medical/religious reasons)
- Hepatitis B (vaccine, titer, or declination form)
- Varicella (titer or 2 vaccines; booster if low titer; declinations only for medical/religious reasons)
- Flu vaccine required annually (religious and medical exemptions accepted with approval and forms)
- Additional health documents:
- Eye exam including visual acuity
- Color blind exam (mandatory, online exam not accepted)
- References: Proof of 2 clinical references or hospital evaluations
- Employment application/resume including licensure, certifications, education, and employment history
